Chronic Disease Self Management Program

The Param CARE Foundation promotes strategies and activities to raise awareness on health maintenance and disease prevention, in a culturally sensitive manner. Healthy behavior among population, through social learning, and thereby get closer to build a healthier community and country is one of the goal of our foundation.

Chronic disease has been characterized as the public health challenges of the 21st century as per the commissioner of the N.J. Department of Health. New Jersey Chronic diseases like cardio vascular diseases, stroke, cancer, arthritis, asthma, diabetes, mental disorders etc. cannot be prevented by vaccination but can be controlled easily by following medication and evidence based self-management program. (CDSMP) Since chronic diseases puts a tremendous burden on individuals, families, communities and the country, because of its devastating effects, physically, mentally, socially and economically management of the chronic diseases are most important. It is proved that chronic diseases prevention and control helps people to live longer, healthier and quality of life.

ParamCARE Foundation is a licensed partner for Chronic Disease Self-Management program with the Division of Aging Services, Department of Human services, State of New Jersey, in the delivery of take control of Health at multi-site in New Jersey, self- management program (CDSMP) since 2014. We are included in the list of licensed partner’s agency and we comply with Stanford University’s requirements for CDSMP delivery and administration. Our master trainers are certified by Stanford University.

We have successfully conducted Stanford University’s Chronic Disease Self-Management Program (CDSMP).at ParamCARE Foundation site from July 26, 2016 to August 31, 2016. This program primarily focuses on healthy lifestyle and how to stay fit even if suffering from chronic disease.. Participants gained knowledge on self-management techniques such as action planning, relaxation, mind distraction, physical exercise, breathing techniques, healthy eating and weight management. Workshop participation certificate was awarded to each participant.

Harvard Medical School Symposium

Param Care Foundation in partnership with Harvard Medical School and S-VYASA has organized a symposium on “Integrative Medicine and role of Yoga and Ayurveda ” Focusing on Emerging world healthcare face challenges of access ,rising cost, standardization and quality control on June 29, 2016.
Mr. Vipul Amin, Director, Param Group of Companies shared his insights on role of Information Technology in education and practice of Yoga and Ayurveda. In addition, he also discussed on technology intervention to understand the concepts of ancient science. Telemedicine is an innovative emerging technology to educate and awareness of healthy daily habits and more on chronic disease management through Yoga and Ayurveda.

International Yoga Day Celebration

ParamCARE Foundation celebrated international yoga day on June 18, 2016 at American Legion hall, Iselin, NJ. Yoga is an ancient physical, mental and spiritual practice that originated in India. The word ‘yoga’ derives from Sanskrit and means to join or to unite, symbolizing the union of body and consciousness.
The United Nations proclaimed 21 June as International Yoga Day.
International Yoga Day aims to raise awareness worldwide of the many benefits of practicing yoga.

8th Annual Pancreatic Cancer Walk

ParamCARE Foundation have outreached the people of various communities, irrespective of their race, ethnicity, cultural, social or economic status, and provided innovative solutions to improve health and enhance the quality of life and well-being socially, physically and emotionally by actively promoting strategies and activities. The organization has organized various events in form of community fairs, health screenings, educational and nutritional seminars and yoga sessions for the local community. We have participated in cancer screening and awareness programs, organized in various locations, for raising awareness on cancer.

Recently, we participated in Pancreatic Cancer on June 11, 2016 which is organized by Middlesex County. Pancreatic Cancer 5K runs for the cure and cancer awareness.

CDSMP program mainly focus on healthy living lifestyle. Participants gained knowledge on self-management techniques such as action planning, relaxation, mind distraction, physical exercise, breathing techniques, healthy eating and weight management. Workshop completion certificates were distributed to each participant.
