Chronic Disease Self Management Program

Param care Foundation believes that health of the people can be ensured by providing the equal opportunity to live in healthy environments, healthy life styles and by the best quality health care. For our outreach activities we have built strong partnerships in the community through trusted and credible organizations like Department of Health and Department of Human Services, Partnership Agreement with the Division of Aging, New Jersey. We are authorized to teach Stanford University’s Chronic Disease Self-Management Program workshops.

We have successfully conducted Stanford University’s Chronic Disease Self-Management Program (CDSMP). Community workshop conducted at Sunny Days Adult Day care center, South Plainfield, NJ, 07080 on May 20, 2015. CDSMP program mainly focus on healthy living lifestyle. Participants learnt self-management techniques such as action planning, relaxation, mind distraction, physical exercise, breathing techniques, healthy eating and weight management. All participants have received their workshop completion certificates.
